Take advantage of the early bird discounted registration fees: register on or before June 30th, 2006 and your registration fee will be $150.00 CDN per registrant.
If you register after June 30th, 2006, the full registration fee will be $175.00 CDN per registrant. Student registration fee is $125.00 CDN with student identification. Registration fees do not include GST 7% and will be added to the registration charge. Due to the limited space, the conference registration will be limited to the first 400 paid registrants.

Registration fees listed above will include one full day of congress sessions, one set of proceedings, morning coffee break, lunch, afternoon coffee break and admission into a post congress reception for registrants (cash bar).
Payment Options
Pay by credit card: complete the form and send the signed copy by mail or fax with your credit card (VISA or MasterCard) information including credit card expiry date.
Pay by cheque: complete the form and mail your cheque payment with your registration form. If your cheque is paying for several registrants, complete one form for each registrant and you may combine the fees for payment with one cheque.
We suggest you make a copy of this completed form for your records.
Confirmation
Registration will only be confirmed once payment has been received.

A discounted room rate has been negotiated per night per room based on singe occupancy. Book early! We cannot guarantee room availability so we encourage you to book early. Payment arrangement for hotel accommodations is your responsibility and will be paid directly to the hotel and is separate from the registration fee for the Congress.
